New Patient Paperwork
As a new patient, you will need to fill out the dental registration & history forms before your first appointment. Please download and print the PDF documents from the links below (you should have a total of 7 forms - Office Policy, Dental Registration & Health History, Notice of Privacy Practices, Acknowledgment of Privacy & New Patient Smile Evaluation). Please answer all fields and bring the completed paperwork with you to your appointment.
Please arrive at least 15 minutes early for your first appointment.
If you have X-Rays at a former dentist's office and would like us to request copies of them, please print and sign the Records Release form and include with your new patient paperwork.
